  • 2025-2026 Season: Added support for the 2025-2026 season, with a season start date of 8/27/2025. The 2025-2026 season rules are aligned with American Academy of Pediatrics recommendations for patients through age 18 years and American Academy of Family Physicians recommendations for children/adolescents and adults. The medical society recommendations indicate individual decision-making for a subset of the population, specifically, previously vaccinated children aged 2 through 18 years not at increased risk, rather than for all individuals aged 6 months and older as indicated by ACIP/CDC. For the 2025-2026 season updates include:

    • Three new 2025-2026 season series:

      • Seasonal 2-dose COVID-19 Series (< 2 years)

  • Added support for CVX 325 (vaccinia (smallpox, mpox), unspecified (NOS)).

  • Removed live vaccine interval for CVX 206. The ICE General Rule "Live Vaccine Interval" rule does NOT apply for CVX 206. CVX 206 does not replicate and for the purposes of timing and spacing recommendations behaves like non-live vaccines.

  • The CVX code specific rule to not enforce the live vaccine interval in the evaluation of CVX 75 and CVX 105 was removed.

  • Updated the CVX 206 name from "Vaccinia, smallpox mpox vaccine live, PF (JYNNEOS)" to “vaccinia, smallpox mpox vaccine live, PF, SQ or ID injection".

Hepatitis A

  • Modified the series selection rules to limit selection of the 3-dose series to when CVX 104 (Twinrix) is administered to an adult patient as dose 1 and/or dose. When Twinrix is not used, ICE will evaluate and forecast in the 2-dose series, where a 6-month interval is required between doses.

  • Added absolute minimum ages and absolute maximum ages to Hepatitis A vaccines. The addition of the absolute maximum age will invalidate pediatric formulations of the vaccines when given to patients 19 years and older, in line with recommendations to repeat the dose. A new CVX-code rule was added so no interval is required between an invalid shot given above the vaccine absolute maximum age and the next target dose.

  • Modified the Hep A 2-dose series: Modified series special rules to allow a grace period between dose 1 and target dose 2 (when invalid between).

  • Modified the Hep A 3-dose series: changes to the absolute minimum age, minimum age, and routine age for target dose 1, added series special rule for Hep A 3-dose series to when CVX 104 (Twinrix) has no maximum age for the Hep A vaccine component CVX 83, added a series special rule for Hep A 3-dose series to when a third dose is not needed.

  • Added PCV20 to the childhood series.

  • Improvements to the  handling of duplicate pneumococcal shots recorded on the same day.

  • For the zip file distribution only: added a new property, disable_output_earliest_and_overdue_dates_for_pneumococcal_adult_series. If set to "Y", ICE will not output the earliest and overdue dates for the Adult Pneumococcal Series. By default, this setting is disabled.

Polio

  • Updated the CVX 02 name from “OPV” to “OPV, trivalent” for additional clarification, in alignment with the CDC CVX description.

COVID-19 - Seasonal

  • Addressed an issue to ensure a vaccine group-level recommendation (rather than CVX-level recommendation) when indicated in the rules for patients in Seasonal COVID-19 < 5 years series. (For patients in an < 5 years series, ICE returns a vaccine group-level recommendation if the patient is >= 5 years as of the recommended date or the evaluation date, and either the recommendation date or the evaluation date is >= 8 weeks from the last administered COVID-19 shot.)

  • Addressed an issue that, in rare circumstances, ICE did not properly select a series in the 2024-2025 season when both the 2023-2024 and 2024-2025 series were completed, resulting in a processing error.

The ICE Implementation Guide Codes Sheet was updated to reflect the addition of a Meningococcal B series, the vaccine name change for CVX 108, the new CVX 328, and the addition of supplemental text descriptive text used in Meningococcal B.

    • Modified the series selection rules for selecting the Novavax series or >= 5 years series, to consider patient age and dose 2 in series selection.

    • Modified the rules to recommend at the CVX level vs group level, to recommend at the CVX level in the Moderna and Pfizer < 5 years series in certain circumstances.

  • Note, for all COVID-19 vaccine seasons, COVID-19 shots administered are evaluated based on the rules in effect as of the shot administration date, and COVID-19 recommendations are based on the rules in effect as of the evaluation date.

Hepatitis B

  • Addressed an issue in the series selection implementation that resulted in the Newborn Series rather than the Child/Adolescent Series being selected under certain circumstances.

Meningococcal B Update

  • Updated ICE to support CVX 316 (Meningococcal MenABCWY (Penbraya)) in the Meningococcal B Vaccine Group.

Given the recurring booster dose recommendation is at the vaccine group level (DTP), this additional information helps clarify the recommendation.

Influenza Update

  • Added support for CVX 320, Influenza, MDCK, trivalent, preservative.

MMR Update

  • Addressed an issue where ICE was not counting a reported indication of immunity when a patient receives one dose of MMR followed by antigens.

  • Addressed an issue where ICE was not returning the correct forecast if 2 or more proof of immunity were documented.

Other

  • A new property, disable_covid19_sep2023_dose_number_reset, was added to the ice.properties file. If enabled, ICE will not reset the dose number to 1 when recommending the Sept 2023 COVID-19 >= 5y series. This option is disabled by default, aligning with the behavior in the last version.

3/18/2024: Release v1.42.1.4

  • Fixes a COVID-19 dose numbering issue to support incrementing of dose number for patients who did not complete a < 5 years series prior to 9/12/2023 and are continuing in a < 5 years series.

  • Clarification to a COVID-19 Sept 2023 COVID-19 Series (>= 5 years) series special rule and corresponding logic update to address an exception related to handling CVX 313 in the >= 5 years series.

  • Extends the "start" date for COVID-19 vaccine from 12/01/2020 to 1/01/1970 to prevent COVID-19 shots administered prior to 12/01/2020 from being assigned to the "Other" vaccine group.

3/07/2024: Release v1.42.1.3

  • Fixes an issue discovered in v1.42.1.2. In certain situations, some historical COVID doses prior to September 12, 2023 were being evaluated as Accepted rather than Valid. This release fixes this issue.

COVID-19 Vaccine Group Rule Sets

  • COVID-19 shots administered before 9/12/2023 are evaluated according to the logic specified in the COVID-19 Pandemic Vaccine Group wiki. Shots administered on or after 9/12/2023 are evaluated according to the logic specified in the new COVID-19 Seasonal Vaccine Group wiki.

  • The two COVID-19 Vaccine Group rule sets are designed to work with one another, with prior history (i.e., before 9/12/2023) influencing current recommendations, as per the ACIP and CDC guidelines.

11/03/2023: Release v1.41.1
